How to get a n95 fit test?

For SON Students and Faculty

The School of Nursing (SON), provides N95 masks to those students who are required to use them. Currently, the SON is supplying Halyard N95 Masks (Regular & Small). In addition, we recently expanded our capabilities to provide fit test opportunities for N95 masks at all our campuses. You are not required to complete your fit test from the SON and you can call your health provider for more options. If you are interested in using the SON fit test, please continue to read below. 

 

The Fit Test Process

Please read all steps and then complete the first step:

  1. Complete the Fit Test Medical Evaluation Questionnaire.

    • After you complete the questionnaire you will receive instructions on how to schedule your appointment if you pass the test.

    • If you fail the test, you may need to complete additional steps. Please see What do I do if I did not pass the OSHA Medical Questionnaire?

    • If you feel that you answered a question incorrectly, you can retake the questionnaire.

    • Do not email any medical information in relation to the questionnaire to our fit test team.

  2. The mask seal will require seal-to-skin contact for a successful fit test. 

  3. Schedule your Fit Test Appointment 

    • Offered at all campuses (Appointments based on availability) 

    • Please review the Student Lab Reminders 

    • Required to show email confirmation from the Fit Test Medical Evaluation Questionnaire

  4. Complete your in-person fit test 

    • You will keep the successfully tested mask 

  5. Receive your fit test record

  6. You may be asked to present your record of fit test. We suggest you make a digital copy.
