This article is for full-time faculty. Every October, the SON Operations Team collects faculty biographical information to complete annual accreditation surveys from the AACN and the BRN. We ask you to complete and review your information to ensure accuracy.

The deadline to complete the Google Form SON Faculty Biographical Information is Friday, October 4, 2024.

• I believe I completed this form last year
• I don't think I have completed this form as I am a relatively new faculty member

When you click the form link, you will see a message saying you have already responded. Good! From here, click on the link “Edit your response.” This will take you to the first page of the form (see below).

Click on the “Next” button. You should see your answers will be pre-populated. Carefully review your answers from the previous year, and make changes to reflect your current status. If no changes are made, select the “Next” button to advance through the form.

When you reach the last page, you will be asked to confirm the year you entered or checked this information. This will help us know that your information is current. For this year’s review, please select “2024” from the drop-down menu. Click the “Submit” button to save your answers.
