Follow these steps to go from the empty BRN pdf to your PHN Certificate!
Please follow the outlined instructions here, which will direct you step-by-step on how to submit your PHN certificate to the BRN alongside your transcripts.
You will follow these steps to send the BRN form and your transcripts.
--> Obtain the PHN form from the BRN site here, pg 7 Section A. Please gather the date you entered your program, the date you received your degree, and what type of Degree you earned. This will be key for Dr. Silao to fill in. Dr. Silao is our associate dean.
You will then email Dr. Silao ([email protected]) to sign section B, and send her your particular program information.
- Once you are emailed the completed BRN form from Dr. Silao, you will then need to request your transcripts here.
- After you fill out your personal information, in the "Recipient" section please select the following 3 options in the appropriate drop-down boxes:
- Who are you sending your transcript to: Education Organization, Application Service, Scholarship and Professional Licensing
- Select Organization: California Board of Registered Nursing
- Department: Advanced Practice Certification (NP, NP Furnishing, NMW, NM
- In the next section, upload your completed BRN form to the "Upload Attachment" section of the form.
- Follow the rest of the steps on this form and submit it.
- After you fill out your personal information, in the "Recipient" section please select the following 3 options in the appropriate drop-down boxes:
- From here, the BRN will review your PHN application and hopefully accept it. They should reach out to you if they need additional information or if there are any issues in certifying you.
